Nelfinavir Drug Combinations to Avoid

Pimozide (Orap)
Pimozide and nelfinavir should not be taken together, due to the risk of dangerous reactions.
 
Proton Pump Inhibitors (PPIs)
Omeprazole can decrease the level of nelfinavir in your blood and should not be combined with nelfinavir. It is not known if this interaction applies to other proton pump inhibitors.
 
Seizure Medications
Some seizure medications can decrease the level of nelfinavir in your blood, perhaps making it less effective. Check with your healthcare provider before taking nelfinavir with a seizure medication. Also, nelfinavir can decrease the level of phenytoin (Dilantin®, Phenytek®) in your blood. Your healthcare provider may need to increase your phenytoin dosage.
 
Some Statin Medications
Nelfinavir should never be combined with lovastatin or simvastatin, due to the risk of dangerous side effects. Since nelfinavir may increase the level of atorvastatin or rosuvastatin in your blood, your healthcare provider may recommend a lower dosage of these medications or may recommend a different statin that does not interact with nelfinavir.
 
St. John's Wort
Because St. John's wort can greatly reduce the level of nelfinavir in your blood, these medications should never be taken together.
 
Trazodone (Desyrel) or Trazodone ER (Oleptro)
Nelfinavir can increase the level of trazodone in your blood. Check with your healthcare provider before taking trazodone with nelfinavir.
 
Warfarin (Coumadin, Jantoven)
Nelfinavir can increase the level of warfarin in your blood, which may lead to an increased risk of bleeding or other serious side effects. When taking nelfinavir and warfarin together, your healthcare provider may decide to monitor your warfarin INR (international normalized ratio) more closely, especially when you are starting or stopping nelfinavir therapy. Your healthcare provider may decide to adjust your warfarin dosage accordingly.
