Prescribing Atazanavir to Adults and Children

Atazanavir Dosage for Adults (Previous HIV Drug Use)

For adults that have taken HIV drugs before ("therapy-experienced" individuals), the recommended dose is atazanavir 300 mg plus Norvir 100 mg all once daily with food. However, a different dose is recommended if you take any of the following medications that can decrease the level of atazanavir in the blood:
 
  • Efavirenz (Sustiva) -- Sustiva and atazanavir should not be combined in therapy-experienced individuals.
     
  • Tenofovir (Viread) -- atazanavir 300 mg plus Norvir 100 mg, all once daily with food. For pregnant women with previous HIV drug use also taking tenofovir, the recommended dosage is atazanavir 400 mg plus Norvir 100 mg once daily with food. 
     
  • H2 blockers, such as cimetidine (Tagamet), famotidine (Pepcid), nizatidine (Axid), or ranitidine (Zantac) -- atazanavir 300 mg plus Norvir 100 mg, all once daily with food, taken at the same time as and/or at least ten hours after the H2 blocker dose. Your healthcare provider may limit how much of the H2 blocker you may take, as well. If you take both an H2 blocker and tenofovir (Viread), the recommend dose is atazanavir 400 mg plus Norvir 100 mg once daily with food, taken at the same time as and/or at least ten hours after the H2 blocker dose. For pregnant women with previous HIV drug use also taking an H2 blocker, the recommended dosage is atazanavir 400 mg plus Norvir 100 mg once daily with food.
     
  • Proton pump inhibitors (PPIs), such as esomeprazole (Nexium), lansoprazole (Prevacid), omeprazole (Prilosec), pantoprazole (Protonix), or rabeprazole (AcipHex) -- PPIs should not be used in combination with atazanavir in therapy-experienced individuals.
     

Atazanavir Dosage for Children

The recommended dose of atazanavir for treating a child with HIV or AIDS depends on body weight and whether they have been on HIV medications before. Atazanavir is often used along with ritonavir (Norvir) in this age group.
